Therapeutic Foster Care
Seraaj Family Homes, Inc., has been a provider of Therapeutic Foster Care (TFC) in Alabama since 1995. Effective October 1, 2008, we have 155 statewide slots.
In TFC, the approach is to meet the individual physical, emotional and social needs of children and youths in a “substitute” family setting until the natural family can be reunited or a permanent placement through adoption can be arranged. The agency designs individualized, needs-driven and family-focused treatment plans for foster children and their families while developing and facilitating a permanency plan for each child. As with our other programs, we have staff available 24 hours a day, seven days a week to Department of Human Resources (DHR) to bring about successful outcomes for youth in the system of care which translates into safety and caring.

Mother and Infant Program
Seraaj Family Homes, Inc. was awarded a contract for the Mother and Infant Program effective October 1, 2007. Currently, we have 19 statewide slots.
Housing and care will be provided to pregnant teens and teen mothers and their infants in specialized foster homes. The program will allow the young mother and her infant to remain in placement after the birth of the child to prepare for independent living or return to family. Pregnant teens and young mothers’ eligible for the program are ages are 13-21. The program will assist with care of the infant while the young mother is developing her skills in parenting and preparing for independent living.

Crisis Stabilization
Seraaj Family Homes, Inc. was awarded a contract for Crisis Stabilization by the State Department of Human Resources (SDHR) to become effective October 1, 2007. Currently, we have 14 statewide slots.
Crisis Stabilization is temporary services to new entries into foster care when the specific placement needs of the child are unknown. Licensed foster parents will provide for the Crisis Stabilization Program. Placements in this program are not to exceed fifteen (15) days unless approved by the Resource Management Division of SDHR. During this fifteen (15) day period, the SDHR will complete a thorough assessment of the child and the family to determine appropriate placement for the child, which may include reunification of the family.

Continuums of Care
Seraaj Family Homes, Inc. has a contract for the Continuums of Care program and was awarded 10 slots in Alabama for Montgomery County, 8 slots in Madison County, and 8 Intensive In Home/Out of Homes services effective October 1, 2008.
The children and families served by the Continuums will have the consistency of a team approach to deliver services in accordance with the Individualized Service Plan (ISP). The team will consist of the supervisor, a family support worker and a therapist from Seraaj Family Homes, Inc., working in intense collaboration with Montgomery County DHR. The team will be in the home of the identified family within 48 hours of referral. The duration of the services is from 9-12 months. Aftercare services are critical for family preservation. We will work as an active ISP team participant to develop an aftercare plan that is specific to the needs of the child and family.

Permanency Project
Seraaj Family Homes, Inc. has a contract for the Permanency Project program and was awarded 10 slots addressing Permanency needs for children in Jefferson County, 25 statewide Reunification Slots and 10 statewide Preservation Slots beginning October 1, 2008.
The children/youth and families served by this project may have a history of the following behaviors to include, but not limited to: aggression, substance misuse, runaway, unruly behavior, problematic sexual behavior, unstable family patterns, stealing/lying, property destruction, and/or serious emotional disturbance. Seraaj Family Homes, Inc. will provide intensive in-home services and therapy to families to facilitate the transition of the children/youth that are ready to return home or to another permanent living arrangement from their out-of-home placement or to maintain children in their own home, when there is a possibility of imminent disruption. Seraaj Family Homes, Inc. team will assist in coordinating the treatment plans of the family and child(ren)/youth and keep the Individualized Service Planning (ISP) team abreast of progress in or barriers to reunification or permanency placement.
