Wrap-around Services"Wrap-around Services" is a term that symbolizes enveloping a client/family with all the services needed to help them meet the challenges of raising children and adolescents with multi-dimensional problems. A Wrap-around support system is a collaborative effort involving agencies and schools to provide a safe, nurturing environment for children and youth who are at risk of being removed from their homes or are in therapeutic foster care. Seraaj Family Homes, Inc. exhausts all efforts to keep children safe at home whenever possible. Wrap-around services, designed to meet the specific individualized needs of clients and families, are available to assist in this effort. Wrap-around Services are also available to foster children and their families. Seraaj Family Homes, Inc. designs individualized, needs-driven and family-focused treatment plans for foster children and their families while developing and facilitating a permanency plan for each child Components of Seraaj Family Homes, Inc. Therapeutic Foster Care and wrap-around Services include: Reunification; Visitation; Individual and Family Therapy; Crisis Intervention; Educational Support; Substance Abuse Prevention and Treatment; Therapeutic Transportation; Step Down; Training in Basic Living Skills; and Documentation. ReunificationReunification services include in-home service to birth families, supervised and unsupervised visitation, family counseling and maintenance checks. Our workers implement the process of reunification whenever possible. VisitationOur workers provide supervision when necessary for visitations between biological families and foster children. Visitation services also include phone contact, pre-approved home visits and pre-approved overnight visits. Individual and Family TherapyIndividual and family therapy services include marital counseling, in-home assessment and problem resolution. The desired result is improved self-esteem and self-reliance and the reduction of needed services. Crisis InterventionSeraaj Family Homes, Inc. workers are available 24-hours per day, 7 days per week for crisis intervention and emergency support. Areas in need of crisis intervention would include conflict resolution and substance abuse counseling. Educational SupportEducational support services include testing, tutoring, classroom behavioral support, vocational assistance, special education, behavioral coaching and transportation. Substance Abuse Prevention and TreatmentSupport services available for clients struggling with substance abuse include assessments, inpatient treatment, aftercare, support groups, individual and family counseling and therapeutic transportation. Therapeutic TransportationTherapeutic transportation is available for school activities, health-related needs, support groups, counseling, employment, assessments, and other activities. Step DownTraining in Basic Living SkillsQualified workers provide support for clients and families in the areas of bill payment, shopping, checking and savings accounts, household expenses and budgeting skills. DocumentationOur staff provides documentation on a monthly basis and on request for ISP's, monthly plans, treatment plans, court reports, incident reports and more.
